
Urinary incontinence that occurs during sexual activity can be embarrassing and causes some men to become anxious or to avoid sex altogether. This urine leakage is stress incontinence, which frequently occurs following prostatectomy or radiation therapy, especially if there is pressure applied to the abdominal area. Although some urine leakage may be a nuisance, urine is sterile and so will not cause infection. To help avoid incontinence during sex, men should make sure their bladder is empty before sexual activity. Some men use a constriction ring to stop urine leakage during sexual activity.
Urinary incontinence may also occur during orgasm. This condition is called climacturia, and it occurs when a man’s internal sphincter does not close properly before orgasm, which allows some urine to leak out. Up to 90 percent of men who undergo prostatectomy will experience at least one episode of climacturia during the early weeks following surgery, but then it rarely recurs.
